Instructions: Respond to each item below by circling the number that best expresses the accuracy of each statement as the description of you and your organization. If the statement is not at all like you and your organization, circle number one. If it fits you perfectly, circle number seven. The numbers in between allow you to indicate varying degrees.

When you have checked all items, review your ratings and then indicate in the box labeled "+" and "-" whether you are satisfied with your rating. Enter a plus (+) if you are pleased with your rating or a minus (-) if you feel a need for a change. The answers will help guide your thinking about changes you may need to make.
